I am gona say D is correct, assuming that the answer given actually means that the on-disk format upgrade is in progress and has yet to complete. I don't think A or B would cause issues with vSAN related GUI menus and the VC would be the first to be upgraded. You would not start the ESXi host upgrade until the VC was complete and all was operating normally.
