I think not all Issues need to be registered in issue register. and some of them can be registered in the daily log

and option D says, only those issues, which raised to the issue register. so D is correct.

I think both are correct: C: The project (under the Change theme) should define how product baselines are created, maintained and controlled. D: Ensure that issues are captured, examined, managed and reviewed throughout the project

Issues do not need to be recorded in the issue register if they have been sufficiently covered in the issue report. The answer is C .
