A new patient presents to the ophthalmologist with a complaint of visual disturbances and decreased vision for one month. He has blind spots in his peripheral visual field. A comprehensive medical eye exam is performed, in addition to fundus photography and an extended visual field examination. The interpretation and report are in the medical record.
