why B and CPT code 46250? Documentation states " each lying along the left lateral column" , wouldn't that warrant selection of 46255? Let alone the ICD10CM codes don't make sense either -- why is the 'correct' answer having diagnosis codes of Unspecified Hemorroids with a diagnosis code of a specified hemorrhoid? The other 2 anothers without K64.9, don't seem to support the documentation either, no where is it stated there is Residual hemorrhoidal skin tags.
